Consultation Fee for Insured Services

This fee is for a consultation with a Nurse Practitioner or Medical Doctor to discuss insurance coverage such as medical botox and to complete related insurance forms and prescriptions.
The fee for this consultation is $75 ($66.37 + HST). This fee cannot be applied towards products or services.
Consultation fees must be paid in advance to secure appointments.
Once this consultation fee payment is received, we will contact you to confirm your appointment.
Please note that ArtMed requires 48 hours notice if you need to cancel or rebook a confirmed appointment. If less than 48 hours notice is provided a $75 cancellation fee will be applied.
